Here's some important information to help you prepare for your visit.
We ask our patients to arrive 15 minutes before their scheduled appointments to allow time for any updates to patient files and other necessary paperwork.
Please bring the following to your appointment:
- Photo ID
- Insurance card(s)
- Co-payment, if needed
- Prescription bottles of all medications, including vitamins and supplements
New Patient: If you are a new patient, please complete this Patient Chart Summary Intake Form for Adults or Patient Chart Summary Intake Form for Pediatrics and bring it with you to your first visit.
Release of Medical Records: If you need to transfer your medical records to/from another medical practice to/from OhioHealth Primary Care Physicians, please complete the Authorization for Use or Disclosure of Protected Health Information (PHI) Form and give it to the medical practice that currently has your records.
Appointment Cancellation and No-Show Policy
We make every effort to schedule an appointment at a time that is convenient for you. As a courtesy to others who may require our services, please call our office as soon as the patient knows he/she needs to cancel the appointment.
Patients will be dismissed from our practice if there are three no-show visits (appointments not cancelled) in a rolling 12-month period of time.
The office prefers that medication(s) is refilled at the office visit. However, if you are running low or are out of your medication, please call the office. The office does has a 48 hour turnaround time for prescription refills.
After-Hours Medical Care
If our office is closed and you need medical care, call our office main phone number at (419) 520.3500 to see if there is a physician on call. If not, and if your medical need is urgent, please go to urgent care or the hospital emergency room.
