A: So that we may provide your care as efficiently as possible, we ask that you bring the following items to your appointment in our office:
A: If you must cancel your appointment, we'll be happy to reschedule at a more convenient time. We ask you to give us 24 hours advance notice in order to provide an opportunity for another patient to be seen. Any patient who fails to show 3 times within a 12 month period will regretfully be discharged from our practice.
A: Patients must pay any co-payments at the time of their visit. Patients are responsible to pay any amounts, such as deductibles, co-payments and co-insurance that their insurance company designates is their responsibility.

If you have questions about your bill for a visit to Dr. Ballinger or Dr. Hanna, call the OhioHealth Physician Group Central Business Office at (614) 544.6366.
A: Please check with your pharmacy to see if you have additional refills remaining on your prescriptions. It is our policy to fill routine prescriptions during scheduled office visits, however you can call the office to request a refill. You may be due for a follow-up appointment if refills are needed. Calls to handle prescription refills can only be handled during office hours. Physicians will not be paged for calls requesting refills.
A: If you are experiencing what you believe is a medical emergency, please call 9-1-1 or go to the closest hospital Emergency Department. To page the on-call physician after-hours for urgent matters, please call our office to be connected to the answering service.
A: The patient is responsible to make sure that the specialist physician is covered on your insurance plan. If your health insurance plan requires a referral form, we will complete the specialty referral paperwork.
A: OhioHealth and our practice are pleased to offer financial assistance to patients with limited resources and inadequate medical insurance coverage. To be considered, you must provide copies of your income and complete an application.
