Our physicians are providers for most major health insurance plans.
Prior to your appointment in our office, it’s a good idea to check if care by our providers is covered by your health plan and to learn about your plan’s covered services, payment terms and referral policy.
Please bring your current insurance card with you to your appointments.
Co-payments are expected to be paid at the time of your appointment. For your convenience, our offices accept MasterCard, Visa, Discover, personal checks and cash as payment for services.
Regardless of your insurance coverage, you are responsible for payment for the services performed. Our staff will be happy to submit initial claim forms to your insurance carrier. If you are unable to show proof of coverage, we expect payment for services at the time of your office visit.
